Athletic Clearance Registration Information

How to register for sports at CHS:

  1. Pick up a Gold Athletic Clearance card from front office.  Please ask Mrs. Walker, front office secretary, for assistants if you cannot locate the cards. Gold card is to keep track of the registration process. Each box must be signed off by The Athletic Directors or Athletic Trainer prior to tryouts. You will hand the gold card to your coach on the first day of tryouts. Without this card completed, you are not allowed to participate in any way until completed. Coaches will not collect any paperwork from athletes.
  2. Go to chs.fuhsd.edu, click on the athletics tab at the top of the page.
  3. On the left hand column, click on “Registration” under Athlete Registration.
    1. This will take you to a link to navigate you to the athletic clearance website. Click on the registration link.
  4. Once on the athletic clearance website watch the “see how it works!” video, it takes 6 minutes and will explain everything in detail.
  5. Returning Athlete Registration Process: (Please use the same login and password all 4 years.)
    1. Sign in, User name: Your Email address Password: (Please write down password)
    2. Click on New Registration at top of the page. Choose current school year, Cupertino, and the sport you are trying out for. Submit.
    3. Step #1 Student information. Click the drop down menu for athlete already in the system. Click on athletes name.
      1. This will auto fill the majority of the registration process. Go through and update any information if necessary.
    4. Step #2 Medical History, update any new injuries or illnesses. Print out Physical Forms.
    5. Step #3 Parent and Guardian Information
    6. Step #4 Parent and Athlete Signatures. Parent need to sign 5 documents and the athletes need to sign 3 of them.
    7. Print out and Sign the signature page. Bring to the athletic trainer.

New Athlete Registration Process:

  1. Click on Register button under the login box.
  2. Fill out the boxes that appear on the right hand of the screen.
  3. Sign in, User name: Your Email address Password: (Please write down password)
  4. Click on New Registration at top of the page. Choose current school year, Cupertino, and the sport you are trying out for. Submit.
  5. Step #1 Student information.
  6. Step #2 Medical History, update any new injuries or illnesses. Print out Physical forms.
  7. Step #3 Parent and Guardian Information
  8. Step #4 Parent and Athlete Signatures. Parent need to sign 5 documents and the athletes need to sign 3 of them.
  9. Print out and Sign the signature page. Bring to athletic trainer.
  1. If you didn’t print out the physical forms during the registration process, you can find the Physicals on the athletics home page under Athletic Participation Forms.
    1. Here are some of the most forgotten information on the physical forms:
      1. Part 1 Athletes Health History: Tetanus shot date. Accurately answers health questions. Signatures of both the parent and athlete.
      2. Part 2 Medical Evaluation: Dr. Clearance at the bottom in grey, check yes or no. Date of the physical, not when it was filled out. The Questionnaire at the top of the physical. Please answer honestly. If you feel this is confidential please hand the physical directly to the athletic trainer.
  2. Sign-up for on-campus sports physicals. They are $20 and will be held on Saturday a few weeks prior to the season.
  3. Those completing sports physicals with their doctor, Bring Completed Physical forms to the athletic trainer. (New policy: Coaches will no longer collect physicals. It must be turned into athletic trainer.)
  4. Attend a concussion screen on campus. (New policy: Beginning for 2017-18 winter sport athlete, this assessment must be completed before tryouts.)
