Student Interventions Process

What is a Student Interventions Request?

A Student Interventions Request is made when a student is not responding to standard classroom interventions, collaborative teacher interventions (shared intervention ideas from the student’s other teachers), and / or contact with parents. Any member of the school staff and/or parent may make this request. This request is submitted to the student’s alpha Guidance Counselor. A meeting between the concerned person and the Guidance Counselor will result in new intervention recommendations or escalation to an Assistant Principal, referral on to the SAT committee, or other options. Please review the Student Intervention Request Process prior to completing a referral form: STUDENT INTERVENTION/STUDENT ASSISTANCE TEAM REFERRAL PROCESS

Once you have reviewed the process and determined a referral is appropriate, fill out the Student Interventions Request (aka SAT referral) found here by clicking the link and saving the form to your computer. Open it Adobe Acrobat and fill it out. Then save the filled out form, and send it to the proper A-Z guidance counselor. DO NOT FILL OUT THE FORM ONLINE. SAVE THE FORM TO YOUR COMPUTER FIRST, THEN FILL IT OUT. If you are having difficulty with this form, please simply print it and submit a hard copy to the student's guidance counselor.
