Transcript Requests


For those who have made your college decision, the next step is to request to have your Final Transcript sent to the ONE college that you will be attending. 

The deadline to request your Final transcript using this form is Thursday, June 10th.

To use this form you must be signed into your FUHSD Google account.  The form is limited to ONE submission.

Transcripts will be sent out AFTER final grades are posted (usually mid-June).

Once submitted this form cannot be changed. If you have any changes (e.g., you are accepted off a waitlist) email the CHS School Registrar at Cheryl_Hassett@fuhsd.org

Steps for requesting an Official CHS Transcript:

1. Visit the CHS Webstore to order an Official Transcript(s) - you will be required to upload your e-receipt onto the CHS Transcript Request Form

2. Complete the CHS Transcript Request Form - this form will need to be completed for EACH official transcript that you (your student) would like to request.  Unofficial transcripts can be accessed in the student's Infinite Campus account.  Prior to completing this form, please visit the Cupertino High School Webstore to purchase the transcript requests.  You will be required to upload your e-receipt to this form.
